Are you a compassionate and dedicated nurse seeking a rewarding career? The Converse Home, a warm and welcoming assisted living community in Burlington, VT, is seeking a per diem nurse to cover open shifts across both our memory care and traditional communities.

The Converse Home is a nonprofit assisted living community dedicated to providing exceptional care in a warm, home-like environment. We are committed to resident well-being and pride ourselves on fostering a supportive, team-oriented workplace where employees are valued and appreciated.

Key Responsibilities:

Provide high-quality nursing care with compassion and professionalism.
Administer medications and treatments in accordance with care plans and physician orders.
Monitor residents’ health status and promptly report changes to the healthcare team.
Collaborate with interdisciplinary team members to develop and implement individualized care plans.
Educate and support residents and their families.
Maintain accurate, timely, and thorough medical documentation.

Qualifications:

Must have a Current RN/LPN license in the state of Vermont.
Experience in assisted living, long-term care, or a related setting preferred.
Strong organizational and time-management skills.
Excellent communication and interpersonal skills.
A team-oriented, patient-centered, and compassionate approach to care.
